Why We Exist

I was born and raised in Vancouver. I did my undergraduate degree at UBC, went to medical school in London, and completed my clinical training and residency at Oxford. Then I came home because this is where I wanted to practise, and this is where I saw a problem worth
solving.

The patients I wanted to help weren’t necessarily sick. Many of them were functioning well, living well, doing everything right. But they were asking questions that the BC healthcare system has no good mechanism to answer.

What is my actual hormonal baseline? Am I at risk for the cardiovascular disease that runs in my family? My energy, my sleep, my recovery — is something off, or is this just age? I feel fine today. Will I still feel fine in ten years?

These are not medical complaints. They are intelligent questions from people who understand that the time to act on health data is before a problem becomes a diagnosis. And the system, through no fault of the people working inside it, is almost entirely unable to help them.

The BC public healthcare model is built to manage disease. It does that — at significant cost, under enormous strain — reasonably well. What it was never designed to do is support proactive, preventive health for people who aren’t yet unwell. Prevention, in the language of public medicine, means a blood pressure check and a conversation about smoking. It does not mean a comprehensive hormone panel. It does not mean understanding your metabolic trajectory, your adrenal function, your cardiovascular risk markers — while you still have years of runway to change the outcome.

And even when a GP genuinely wants to go further — and many do — their hands are often tied by what MSP will fund. The tests that matter most for early detection and prevention are frequently the ones that fall outside public coverage. Comprehensive hormone panels, advanced thyroid markers, detailed cortisol rhythms, metabolic and inflammatory markers — these require either an existing diagnosis to justify them, or the patient to pay out of pocket through a system that isn’t set up for that either. The result is a gap between what medicine can tell you and what the public system will actually measure.

Then there is the reality of what GPs are being asked to carry. The family physicians in BC are not failing their patients — they are working in a system that is failing them. A roster of two thousand patients. Administrative burden that eats hours every day. Ten-minute appointments to address whatever has been worrying someone for months. The thorough, unhurried, data-driven conversation that preventive medicine actually requires doesn’t fit in that window, and it isn’t fair to anyone to pretend it does.

Sapere Health exists for the space the public system cannot reach.

I built this because I believe that data-driven preventive health should not be a privilege. Every British Columbian who wants to understand their body before something goes wrong deserves access to a physician who will take that seriously.

She was 41, active, and had been exhausted in a way that sleep didn’t fix for nearly two years. Her GP had run standard panels twice. Both times the results came back within range. She was told it was likely perimenopause, possibly stress, and to focus on sleep hygiene.

What the standard panels hadn’t captured was the full hormonal picture. When I ran a comprehensive female hormone panel, her free testosterone was critically low — not flagged, but well below functional for her age and activity level. Her estradiol was declining faster than expected. Her cortisol pattern showed a blunted morning peak with an abnormal afternoon rise — a pattern consistent with chronic physiological stress that her body had stopped signalling clearly.

None of these results were individually outside the broad population reference range. Together, they told a coherent and actionable story.

Within four months of targeted interventions guided by the data, she described herself as functional again — training consistently, sleeping deeply, thinking clearly.

Who Is Behind Your Results

Dr. Ryan Kingan is a Vancouver-born physician who completed his undergraduate education at the University of British Columbia, his medical degree in London, and his clinical training and residency at the University of Oxford. He is licensed with the College of Physicians and Surgeons of British Columbia (CPSBC) and practises in Vancouver.

Every Sapere test is physician-ordered under Dr. Kingan’s licence. Every result is reviewed by him personally before delivery. All laboratory analysis is conducted at CLIA-certified, CAP-accredited facilities.
